That is absolutely terrible.  We can all arm chair qb this one but it won't bring her back so I won't state the obvious.  I have been described as "a bit much" with protocols and safety on the boat, but I will take that over this situation any day.  My thoughts and prayers are with this family.   

Don't get complacent out there folks.  It only takes a second and lives are changed forever.  This shows us all that the worst can happen if you slip up for just a second.  Truly sad situation.
